Welcome to the Auburn Village School! To register a child for the 2024-2025 or the 2025-2026 school year for grades Preschool – 8, please complete this online form. Choose the enrollment year 2025-2026 for next school year!
A student may enter grade one if his/her chronological age will be six before September 30 of the year of entering school. A child entering kindergarten must be five years of age on or before September 30. A birth certificate must be presented upon registration as proof of the date of birth. Our policy is here.
- The Child’s Birth Certificate
- Picture ID of Parent/Guardian
- 2 Proof of Residency (examples include a current Utility Bill, Tax Bill, Mortgage Statement, Purchase and Sale Agreement, Certificate of Occupancy, or a Lease/Rental Agreement)
- Residency Affidavit – only if living with another family (please ask office for form)
- The Child’s Immunization Record
- Documentation of the child’s last physical exam (must have occurred on or before September 1st for the start of a new school year)
- Court Order/Custody Agreement – Parenting Plan
The new student registration packet includes the following forms:
- Student Registration Form
- Home Language Survey
- Authorization to Release Student Records
- Certification of Residency
- Student Residency Questionnaire
- Free and Reduced Lunch Application
Please note that if your child requires medication to be given in school including over the counter medications such as Tylenol, Advil, cough/cold medications, etc., we must have a note from the parent/guardian as well as a note from the health care provider. Please call the school nurse at 483-2769 ext. 1107 if you have any questions regarding medications, physicals or immunization requirements.
