Kindergarten Registration Packets will be available at the main office beginning Monday, January 11^th. Completed registration information packets are due in the main office by Friday, January 29th. All children living in Auburn who will be five years old on or before September 30th, 2021 are eligible to enter kindergarten in the fall of 2021. To streamline registration, please complete this online preregistration survey. There will be a kindergarten parent information meeting in the spring. We will need the following information returned with each packet:

• The Child’s Birth Certificate
• Picture ID of Parent/Guardian
• 2 Proof of Residency (examples include a current Utility Bill, Tax Bill, Mortgage Statement, Purchase and Sale Agreement, Certificate of Occupancy, or a Lease/Rental Agreement)
• Residency Affidavit – only if living with another family (please ask office for form)
• The Child’s Immunization Record
• Documentation of the child’s last physical exam (must have occurred on or before September 1st for the start of a new school year)
• Court Order/Custody Agreement – Parenting Plan

If your child has health needs that require special assistance during school hours or you have any significant concerns about your child’s health or development, our School Nurse is available for consultation.  Please schedule an appointment with her by phone at 483-2769 ext. 1107.

Please note that if your child requires medication to be given in school including over the counter medications such as Tylenol, Advil, cough/cold medications, etc., we must have a note from the parent/guardian as well as a note from the health care provider. Please call the school nurse at 483-2769 ext. 1107 if you have any questions regarding medications, physicals or immunization requirements.

If you are registering your child to begin school at a time other than the start of a new school year, there will be additional forms for you to complete once you return all documents listed above.
